Position Purpose: Supports care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Interacts with members by performing member outreach…

You will work closely with a small, friendly team and take on more as you settle in. Most days start with a short catch-up, then plenty of time for the work itself.

• Look after the day-to-day running of your area
• Work with the people around you to plan the week ahead
• Share what you learn and help newer members of the team
• Keep things tidy, on time and to a high standard
